All employees of BioHorizons working a minimum of 30 hours per week (excluding seasonal and temporary employees) are eligible to participate in a wide range of benefits.

comprehensive medical plan
Eligibility begins on the first day of the month following date of hire.

dental plan
Includes preventative, basic and major care benefits.

employee assistance program
Gives employees and their family members the opportunity to speak to a professional counselor confidentially about any type of personal concern that may be affecting their work or professional life.

voluntary supplemental coverage
Available for cancer and other critical illnesses.

voluntary vision plan
Available for purchase.

paid sick leave
Granted at the beginning of each year. A certain amount may be carried over from year-to-year.

Short term and long term disability insurance coverage is available to all full-time employees. Disability insurance provides partial income protection when an employee is unable to work due to illness or injury.

flexible spending accounts (FSA)
Enable employees to set aside pre-tax dollars for certain health care and dependent care expenses. Each dollar you put into an FSA account is a dollar not taxed. A convenient Benefits Debit Card is available to pay eligible expenses that are automatically withdrawn from your account.
vacation and holidays
BioHorizons understands the need to have time off from work to relax and spend time with family. We encourage employees to take paid vacation time to rejuvenate and have fun.

holidays and personal days
Key holidays can be taken as paid holidays. Paid personal days are provided for other dates important to you.

BioHorizons urges every employee to prepare for his/her retirement. The BioHorizons 401(k) program provides a foundation for retirement income and includes a company matching program.

life insurance
BioHorizons provides a basic group term life / accidental death & dismemberment (AD&D) plan. Employees and eligible spouses may also elect additional voluntary supplemental life coverage.

voluntary AD&D Insurance
Employees who participate in the voluntary supplemental life plan may also elect voluntary AD&D coverage for themselves and/or their dependents.

The BioHorizons Health Plan provides this link to access machine readable files as required by the Transparency in Coverage Rule. The files include negotiated service rates and out-of-network allowed amounts between this plan and healthcare providers.
